Submit Your Student Health Information & Immunization Records

  • Chatham is committed to the health and wellbeing of all of our students. All full-time students are required to have health insurance coverage.

Full-time students who wish to enroll in the Chatham University Student Health Insurance Plan can find more information online at myChatham. The University-sponsored plan is designed to meet basic health care needs at an affordable cost.

Any questions should be emailed to the Office of Student Accounts.

  • All students are required to submit health information and immunization record information through a secure web-based software platform called Med+Proctor, which will become part of your health record in the Office of Student Health Services. Check your Chatham University email for the link to create a Med+Proctor account.

Med + Proctor Vaccine Upload Instructions

    1. Visit secure.medproctor.com and click New Users-Register

    1. Register with your Chatham email and complete registration.

    2. When approaching the documents area, if asked to pay $10 for a Pro Account, click No Thank You. Med+Proctor is a free service and the Pro account is not needed at any point of the uploading process.

    3. Med+Proctor provides immunization certificates to download and give to your PCP to fill out. It is not required to submit immunizations on the certificate, as the certificate is there only if needed. Chatham requires all undergraduate students to upload documentation of the following records to Med+Proctor:

2 doses MMR vaccine or positive titers for Measles, Mumps, and Rubella

    3 doses Hepatitis B or a positive Hepatitis B surface antibody blood-test

1 dose Meningitis A vaccine (1 dose is required AFTER age 16)

Tuberculosis screening questionnaire (Follow-up testing required if screening is flagged)

Waivers are also available for download underneath the documents section.
